Physicians who participate in this program offer members a minimum discount of 25 percent off covered charges. As a CCA member, you may change doctors whenever you choose. You will receive a discount as long as you are treated by a physician who participates in CCA. When you enroll in the program, you will be asked to list the names of your immediate family on the enrollment form. All family members receive the same membership card number. Simply present your membership card at sign-in, and your membership will be confirmed.
Provider Directory Terms of Use
Physicians who have chosen to participate in the program are independent providers and are not employed by CHRISTUS Hospital or CHRISTUS Health. Discounts are applicable only when payment is made at the time of services. The program is subject to change or withdrawal at any time. If you obtain health insurance coverage, the pricing and terms of your insurer's provider agreement will apply. This directory is provided by CHRISTUS Care Advantage (CCA) for use by the general public as a quick reference of information about physicians who participate in the CCA discount program. The Directory is not intended as a tool for verifying the credentials, qualifications, or abilities of any physician contained therein. Inclusion in the Directory does not imply recommendation or endorsement. You are prohibited from using, downloading, republishing, selling, duplicating, or "scraping" for commercial or any other purpose whatsoever, the Provider Directory or any of the data listings or other information contained therein, in whole or in part, in any medium whatsoever. The Provider Directory is provided on an "AS-IS" basis. The database of physician information which drives WebMD Physician Directory does not contain sufficient information with which to verify physician credentials under the standards of the Joint Commission, National Committee for Quality Assurance (NCQA) of the Utilization Review Accreditation Committee (URAC).
